၅။ မြန်မာနိုင်ငံ အိမ်ထောင်စုစာရင်းမိတ္တူ ၁ စုံ
၆။ နော်ဝေနိုင်ငံမှ ထုတ်ပေးသော နိုင်ငံကူးလက်မှတ်မိတ္တူ(ကာလာ) ၁ စုံ (National ID number ပါ စာမျက်နှာ) (သို့မဟုတ်) နိုင်ငံသားဖြစ်ကြောင်း အထောက်အထား

မှတ်ချက်။ နိုင်ငံသားအဖြစ်မှ စွန့်လွှတ်ခွင့် လျှောက်ထားသူသည် မိမိ၏ မူရင်းနိုင်ငံကူးလက်မှတ် နှင့် မြန်မာနိုင်ငံသားမှတ်ပုံတင်ကတ်ပြားတို့ကို ပြန်လည်အပ်နှံရမည်ဖြစ်သည်။

1. Personal Application Form 1 Copy
2. CV Form (with photo attached) 1 Set
3. Original Myanmar Passport 1 Set
4. Original Myanmar National Registration Card (NRC) 1 Set
5. Myanmar Household Members List 1 Set
6. Copy of Valid Norwegian Passport (page containing National 1 Set ID number) or Proof of Norwegian Citizenship

Note: Applicants applying for renunciation of citizenship must surrender their original Myanmar passport and Myanmar National Registration Card.
